Medical nutrition therapy is essential to health and quality of life. Individuals with a variety of conditions and illnesses can benefit from a personalized nutrition consultation with the Healthy Hearts Center’s outpatient nutritionist.

Why nutrition matters
Proper nourishment is important in building and maintaining the immune system and the overall health of our bodies. Achieving wholesome nutritional balance prevents malnutrition and is fundamental in maintaining energy and good health.
Many medical conditions either develop or are made worse by improper or unhealthy eating habits. Our goal is to provide expertise in nutrition for the optimal health of our patients. What we eat can be a social aff air and for that reason, information will be provided for you to refer to and share with your friends, family members, and loved ones.
Your nutrition consultation
The nutrition care process includes assessment, creation of patient’s self-management goals, development of a nutrition plan, documentation, communication with the primary care provider, evaluation and reassessment as well as outcome measurements.
Please bring the following to your consultation:
- A list (or actual bottles) of all current medications, including herbal supplements and vitamins
- A three-day food record
- A close friend of family member, if desired (especially if they help prepare meals or with grocery shopping)
It is important that we communicate with your primary care physician. We ask that you contact your physician’s office and provide any relevant medical information such as recent lab results, medication lists, or medical history highlights that would allow us to better assess your nutritional needs. All information can be faxed to the outpatient nutritionist at 317-621-8571.
